Consent Form: Eating insects and other invertebrates

    Your child’s forthcoming trip to Paignton Zoo involves a session looking at the possibility of insects as a sustainable food source.

     

    Some of the students will take part in a challenge to see if they are prepared to eat insects and other invertebrates. The insects will be dead and freeze dried. They have been sourced from a reputable supplier and are produced, packaged and sold for human consumption in an environment that does contain nuts.

     

    If your child has a nut / mollusc / shellfish / dust mite / insect sting allergy or severe asthma, they should NOT eat insects.

     

    Please give permission for your child to take part in this activity by ticking the boxes below and providing the details requested.

    If your child does have any of the allergies listed above, and/or you do not wish to give permission for your child to eat insects, you do not have to submit this form. We will not permit any learners who have not had a form completed to partake in this activity.
